ABSTRACT

Arboviruses cause public health problems in several countries, and records show that they can generate central and peripheral neurological complications with permanent sequelae. However, it is not certain which arbovirus is responsible for outbreaks of the Guillain-Barré Syndrome (GBS), especially in Brazil. Thus, the objective of this study is to verify if there is a coincidence between the GBS outbreak and the most common arboviruses in Northeastern Brazil, as well as their relationship. An ecological time series study was designed with the federative units of Northeastern Brazil, using hospitalizations for Guillain-Barré syndrome and notifications of arbovirus infections between 2014 and 2019 as a data source. Distribution incidence curves were constructed for the conditions studied, and generalized estimating equations (GEE) models were applied to estimate the relationship between arboviruses and Guillain-Barré. The results showed a similar distribution for the incidences of Chikungunya virus (z=7.82; p=0.001), Zika virus (z=3.69; p=0.03), and Guillain-Barré syndrome (z=2.98; p=0.05) from 2014 to 2019. The GEE model revealed that the distribution of Chikungunya incidence is associated with the distribution of GBS incidence in each year (x2Wald=3,969; p=0.046). This pattern was repeated in seven of the nine states, while the Zika virus had a significant relationship with GBS in only two states. The outbreak of GBS in Northeastern Brazil appears to be probabilistically related to outbreaks of the Chikungunya virus.


As arboviroses são problemas de saúde pública em vários países e há registros de que podem produzir complicações neurológicas centrais e periféricas com sequelas permanentes. Entretanto, não se sabe ao certo qual delas é realmente responsável pelos surtos da Síndrome de Guillain-Barré (SGB), principalmente no Brasil. Assim, o objetivo é verificar se há coincidência entre o surto de SGB e as arboviroses mais comuns no Nordeste do Brasil e suas relações. Foi desenhado um estudo ecológico de série temporal com as unidades federativas do Nordeste do Brasil, adotando como fonte de dados as internações Guillain-Barré e as notificações de infecções por arbovírus entre 2014 e 2019. Curvas de distribuição de incidência foram construídas para as condições estudadas, e foram aplicados modelos de equações generalizadas estimadas (GEE) para estimar a relação entre arbovírus e Guillain-Barré. Evidencia-se que há distribuição semelhante para as incidências do vírus Chikungunya (z=7,82; p=0,001), vírus Zika (z=3,69; p=0,03) e síndrome de Guillain-Barré (z=2,98; p=0,05) entre 2014 e 2019. O modelo GEE revelou que a distribuição da incidência de Chikungunya está associada à distribuição da incidência de SGB em cada ano (x2Wald=3,969; p=0,046). Esse padrão se repetiu em sete dos nove estados, enquanto o zika vírus teve uma relação significativa com o GBS em apenas dois estados. Conclui-se, então, que o surto de SGB no Nordeste do Brasil parece estar probabilisticamente relacionado aos surtos do vírus Chikungunya.


Los arbovirus causan problemas de salud pública en varios países y, según indican los reportes, pueden producir complicaciones neurológicas centrales y periféricas con secuelas permanentes. Sin embargo, no se sabe cuál de ellos es realmente el responsable de los brotes del síndrome de Guillain-Barré (SGB), especialmente en Brasil. Así, el objetivo de este estudio es verificar si existen coincidencias entre el brote del SGB y los arbovirus más comunes en el Noreste de Brasil y sus asociaciones. Se diseñó un estudio de series temporales ecológico en las unidades federativas del Noreste de Brasil, adoptando como fuente de datos las hospitalizaciones y las notificaciones de arbovirosis de Guillain-Barré entre 2014 y 2019. Se construyeron curvas de distribución de incidencia para las condiciones científicas, y se aplicó una ecuación estimada generalizada (GEE) para estimar la relación entre arbovirus y Guillain-Barré. Se encontró que existe una distribución similar en las incidencias de virus del chikunguña (z=7,82; p=0,001), virus del Zika (z=3,69; p=0,03) y síndrome de Guillain-Barré (z =2,98; p=0,05) entre 2014 y 2019. El modelo GEE reveló que la distribución de la incidencia de chikunguña está asociada con la distribución de la incidencia de SGB en cada año (x2Wald=3,969; p=0,046). Este patrón se repitió en siete de los nueve estados, mientras que el virus del Zika presentó una relación significativa con el SGB en solo dos estados. El brote del SGB en el Noreste de Brasil parece estar relacionado probabilísticamente con los brotes del virus del chikunguña.

Abstract Background Chikungunya is a mosquito-borne disease caused by the chikungunya virus (CHIKV) and can lead to neurological complications in severe cases. Objective This study examined neuroimaging patterns in chikungunya cases during two outbreaks in Brazil to identify specific patterns for diagnosis and treatment of neuro-chikungunya. Methods Eight patients with confirmed chikungunya and neurological involvement were included. Clinical examinations and MRI scans were performed, and findings were analyzed by neuroradiologists. Data on age, sex, neurological symptoms, diagnostic tests, MRI findings, and clinical outcomes were recorded. Results Patients showed different neuroimaging patterns. Six patients exhibited a "clock dial pattern" with hyperintense dotted lesions in the spinal cord periphery. One patient had thickening and enhancement of anterior nerve roots. Brain MRI revealed multiple hyperintense lesions in the white matter, particularly in the medulla oblongata, in six patients. One patient had a normal brain MRI. Conclusion The "clock dial pattern" observed in spinal cord MRI may be indicative of chikungunya-related nervous system lesions. Isolated involvement of spinal cord white matter in chikungunya can help differentiate it from other viral infections. Additionally, distinct brainstem involvement in chikungunya-associated encephalitis, particularly in the rostral region, sets it apart from other arboviral infections. Recognizing these neuroimaging patterns can contribute to early diagnosis and appropriate management of neuro-chikungunya.


Resumo Antecedentes A chikungunya é uma doença transmitida por mosquitos causada pelo vírus chikungunya (CHIKV) e pode levar a complicações neurológicas em casos graves. Objetivo Este estudo examinou padrões de neuroimagem em casos de chikungunya durante dois surtos no Brasil para identificar padrões específicos para o diagnóstico e tratamento de neurochikungunya. Métodos Oito pacientes com chikungunya confirmada e envolvimento neurológico foram incluídos. Exames clínicos e ressonâncias magnéticas (RM) foram realizados, e os achados foram analisados por neurorradiologistas. Dados sobre idade, sexo, sintomas neurológicos, testes diagnósticos, achados de RM e desfechos clínicos foram registrados. Resultados Os pacientes apresentaram diferentes padrões de neuroimagem. Seis pacientes apresentaram um "padrão de mostrador de relógio" com lesões pontilhadas hiperintensas na periferia da medula espinhal. Um paciente apresentou espessamento e realce das raízes nervosas anteriores. A RM do cérebro revelou múltiplas lesões hiperintensas na substância branca, especialmente no bulbo em seis pacientes. Um paciente apresentou uma RM cerebral normal. Conclusão O "padrão de mostrador de relógio" observado na RM da medula espinhal pode ser indicativo de lesões do sistema nervoso relacionadas à chikungunya. O envolvimento isolado da substância branca da medula espinhal na chikungunya pode ajudar a diferenciá-la de outras infecções virais. Além disso, o envolvimento distinto do tronco cerebral na encefalite associada à chikungunya, especialmente na região rostral, a distingue de outras infecções por arbovírus. O reconhecimento desses padrões de neuroimagem pode contribuir para o diagnóstico precoce e manejo adequado da neurochikungunya.

Abstract Objectives To identify and understand the evidence regarding hearing changes related to acquired Dengue, Chikungunya, and Zika virus infection in adult individuals. Methods A scoping review was performed according to the recommendations of The Joanna Briggs Institute and guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews in the Embase, PubMed/Medline, ScienceDirect, Scopus, and Web of Science databases without restriction on language and year of publication. Case studies, observational studies, and clinical trials reporting hearing loss in adult subjects (>18-60 years of age) of both sexes with DENV, CHIKV, or ZIKV diagnosed by positive molecular/serological examination by RT-PCR or IgM/IgG by ELISA method were included. Results Thirteen studies met the inclusion criteria and were selected for review. The occurrence of auditory symptoms caused by arboviroses and the presence of permanent or transient sensorineural hearing loss was variable in adults. Conclusions Dengue, Chikungunya, and Zika infections in adults are associated with a variety of auditory symptoms. The frequency of permanent or transient sensorineural hearing loss is low but not negligible.

Introducción: La infección por Chikungunya se presenta fiebre y afectación cutánea. Las manifestaciones neurológicas, incluyen encefalopatías principalmente encefalitis; afectación periférica como mielitis, o una combinación de éstas. Objetivo: Describir la frecuencia y las manifestaciones neurológicas asociadas a infección por virus Chikungunya en el periodo enero-marzo 2023 en una población pediátrica. Materiales y Métodos: Estudio descriptivo retrospectivo de corte transversal de serie de casos Ingresaron menores de 18 años con manifestaciones neurológicas y resultados positivos PCR RT a virus Chikungunya que acudieron a un Hospital Público en enero-marzo del 2023. Las variables: demográficas, tiempo de evolución, síntomas, diagnósticos neurológicos, estudio de líquido cefalorraquídeo, electroencefalograma, estudios imagenológicos, tratamiento, ingreso a Unidad de Cuidados Intensivos, disfunción orgánica, mortalidad. Los datos se analizaron en SPSS utilizando estadística descriptiva. El protocolo fue aprobado por el comité de ética. Resultados: Ingresaron 24 pacientes, con edad de 10.0 ±1 meses. El 58.7% de sexo masculino. Como síntoma neurológico, el 54.1% tuvo convulsión. Los diagnósticos neurológicos, el 83.3% fue Encefalitis. Los pacientes con diagnóstico de Encefalitis, 75% fueron menores de 3 meses, 50% con líquido cefalorraquídeo patológico, 45% se realizó Electroencefalografía, 50% recibió inmunoglobulinas. El 50% ingresaron a Unidad de Cuidados Intensivos. Pediátricos. El 60% presentó disfunción orgánica. La mortalidad fue del 4.2%. Conclusión: El diagnóstico neurológico más frecuente fue la encefalitis, predominó en lactantes menores de 3 meses. Los síntomas neurológicos fueron: crisis convulsivas e irritabilidad. Más de la mitad presentaron disfunción orgánica, se registró la mortalidad de un paciente.


Introduction: Chikungunya infection present clinically with fever and skin involvement. Neurological manifestations include encephalopathies, mainly encephalitis and meningoencephalitis; peripheral involvement such as myelitis, Guillain Barré Syndrome; or a combination of these such as encephaloneuromyelopathy. Objective: To describe the frequency and neurological manifestations associated with Chikungunya virus infection during the January-March 2023 time period in a pediatric population. Materials and Methods: This was a descriptive, retrospective and cross-sectional study of a case series. Minors under 18 years of age were admitted with neurological manifestations and positive RT-PCR results for Chikungunya virus who presented to a Public Hospital in January-March 2023. The variables were: demographics, reason for hospitalization, symptoms, neurological diagnoses, cerebrospinal fluid study, electroencephalogram, imaging studies, treatment, admission to the Intensive Care Unit, organic dysfunction and mortality. Data were analyzed in SPSS using descriptive statistics. The protocol was approved by the Ethics Committee. Results: 24 patients were admitted, aged 10.0 ±16 months. 58.7% were male. As a neurological symptom, 54.1% had a seizure. Among the neurological diagnoses, 83.3% were encephalitis. Among the patients diagnosed with encephalitis, 75% were younger than 3 months, 50% had pathological cerebrospinal fluid, 45% underwent electroencephalography, and 50% received immunoglobulins. 50% were admitted to the Pediatric Intensive Care Unit. 60% presented organic dysfunction. Mortality was 4.2%. Conclusions: The most frequent neurological diagnosis was encephalitis, it predominated in infants under 3 months. The neurological symptoms were: seizures and irritability. More than half presented organic dysfunction, one patient expired.

Introducción: La Enfermedad por el virus del Chikungunya (CHIKV) es una virosis que se caracteriza por fiebre acompañada de dolores articulares. La epidemia de esta enfermedad en los últimos dos años causó más de cien mil casos confirmados y sospechosos, incluido personas con Lupus Eritematoso Sistémico (LES). Objetivo: Los objetivos del siguiente trabajo consistieron en determinar la evolución de la Enfermedad por el CHIKV en pacientes con LES, caracterizar a la población incluida en el estudio, determinar la frecuencia de la Enfermedad por el CHIKV en pacientes con LES, describir las manifestaciones clínicas de la Enfermedad por el CHIKV en pacientes con LES y registrar la impresión de los pacientes en relación con la actividad de la enfermedad y las manifestaciones clínicas que aparecieron posterior a la Enfermedad por el CHIKV. Metodología: El estudio realizado fue de tipo descriptivo de corte transversal. Se incluyeron 101 pacientes con Lupus Eritematoso Sistémico, a quienes se les encuestó mediante un formulario virtual. Los investigadores establecieron contacto con los pacientes a través de llamadas telefónicas o mensajería, y si los pacientes aceptaban participar en el estudio, se les enviaba el formulario o, en algunos casos, se completaba durante la llamada telefónica, si el paciente lo solicitaba. Resultados: El valor de la media de edad es de 38,2 ± 11,8 años. El 85,1% (86/101) de los pacientes corresponden al sexo femenino, mientras que el 14,9% (15/101) corresponde al sexo masculino. El 37,6% (38/101) presentó la enfermedad por el CHIKV, la cual se caracterizó en la fase aguda por la presencia de artralgias en un 84,2% (32/38), fiebre en un 71,1% (27/38), debilidad en un 57,9% (22/38) y cefalea en un 55,3% (21/38). En la fase subaguda, las artralgias se manifestaron mayoritariamente en un 73,7% (28/38) de los pacientes. La fase crónica estuvo presente en el 71,1% (27/38) con la persistencia de artralgias en un 81,48% (22/27) de los pacientes acompañada de otros síntomas como la artritis en un 40,74% (11/27). A causa de la enfermedad por el CHIKV, el 21,1% (8/38) recibió la indicación en la fase subaguda o crónica de realizar cambios en su medicación para el LES. El 62,5% (5/8) recibió la indicación de aumentar la dosis del corticoesteroide, mientras que el 50% (4/8) de aumentar la dosis del inmunosupresor. Según la impresión de los pacientes, en cuanto a la actividad de la enfermedad de base, posterior a la enfermedad por el CHIKV, presentaron una exacerbación del LES en un 10,5% (4/38) de los pacientes, de los cuales un 50% (2/4) refirieron exacerbación de síntomas ya existentes antes de la infección y la aparición de nuevos síntomas en un 50% (2/4). El 5,2% (2/38) tuvo requerimiento de hospitalización debido a la enfermedad por el CHIKV en la fase aguda. Conclusión: Las artralgias fueron frecuentes en las fases aguda, subaguda y crónica. En términos de tratamiento, algunos pacientes ajustaron sus medicaciones para el LES, aumentando la dosis de inmunosupresores o corticoides. En cuanto a la evolución, la mayoría de los pacientes no experimentaron una exacerbación de la enfermedad de base debido a la enfermedad por el CHIKV, al igual que solo un par de pacientes requirió hospitalización.


Introduction: Chikungunya virus (CHIKV) disease is a viral disease characterised by fever accompanied by joint pain. The epidemic of this disease in the last two years has caused more than 100,000 confirmed and suspected cases, including people with systemic lupus erythematosus (SLE). Objective: The objectives of the following work were to determine the evolution of CHIKV disease in patients with SLE, to characterise the study population, to determine the frequency of CHIKV disease in patients with SLE, to describe the clinical manifestations of CIHKV disease in patients with SLES, and to record the patients' impression of disease activity in relation to CHIKV disease in patients with SLE. Methodology: The study was a descriptive cross-sectional study. It included 101 patients with Systemic Lupus Erythematosus, who were surveyed using a virtual form. The investigators contacted patients by phone calls or messages, and if patients agreed to enter the study, the form was sent to them or, in some cases, completed during the telephone call, if requested by the patient. Results: The mean age value is 38.2 ± 11.8 years. The female sex accounted for 85.1% (86/101) of the patients, while 14.9% (15/101) were male. Chikungunya virus disease occurred in 37.6% (38/101) and was characterised in the acute phase by arthralgias in 84.2% (32/38), fever in 71.1% (27/38), weakness in 57.9% (22/38) and headache in 55.3% (21/38). In the subacute phase, arthralgias manifested in 73.7% (28/38) of patients. The chronic phase was present in 71.1% (27/38) with persistence of arthralgias in 81.48% (22/27) of patients accompanied by other symptoms such as arthritis in 40.74% (11/27). Due to CHIKV disease, 21.1% (8/38) received an indication in the subacute or chronic phase to make changes in their medication for SLE. 62.5% (5/8) received an indication to increase the dose of corticosteroid, while 50% (4/8) received an indication to increase the dose of immunosuppressant. According to the patients' impression of the baseline disease activity following CHIKV disease, patients reported an exacerbation of SLE in 10.5% (4/38) of patients, of whom 50% (2/4) reported exacerbation of symptoms already existing before infection, the appearance of new symptoms in 50% (2/4). Only 5.2% (2/38) referred to hospitalization due to CHKV disease in the acute phase. Conclusion: Arthralgias were frequent in the acute, subacute, and chronic phases. In terms of treatment, some patients adjusted their SLE medications, increasing the dose of immunosuppressants or corticosteroids. In terms of evolution, most patients did not experience an exacerbation of their baseline disease due to CHIKV disease, and only a couple of patients required hospitalization.

La fiebre Chikungunya es causada por un arbovirus ARN perteneciente a la familia Togaviridae del género alphavirus, llamado "virus del chikungunya", este virus tiene como vector el Aedes aegypti y el Aedes albopictus. En su fase aguda la enfermedad se manifiesta por fiebre, artritis o artralgias severas, mialgias, cefalea, fotofobia, linfadenopatías y brotes cutáneos, los síntomas pueden progresar a una fase subaguda hasta crónica, donde persisten las manifestaciones articulares y la fatiga. Se realizó una revisión sistemática en las bases de PubMed, Cochrane, Dovepress y SciELO, utilizando las palabras claves: chikungunya y chikunguña, cruzadas con las palabras: artritis crónica y tratamiento. Se incluyeron en el estudio un total de 16 artículos, 6 artículos de revisión, 4 artículos originales, 2 ensayos clínicos, 2 reportes de caso, 1 revisión sistemática y 1 carta al editor. Entre las opciones farmacológicas, el metotrexato es el fármaco más estudiado y compite con los efectos de los AINE. Como terapias no farmacológicas se encuentran la fisioterapia y la terapia física, utilizados como complementos a la terapia farmacológica. El uso de terapias caseras, incluso la homeopatía, también podrían agregarse, atendiendo principalmente el factor psíquico y cultural de los pacientes.


Chikungunya fever is caused by an RNA arbovirus belonging to the Togaviridae family of the alphavirus genus, called "chikungunya virus", this virus is vectored by Aedes aegypti and Aedes albopictus. In its acute phase, the disease is manifested by fever, arthritis or severe arthralgia, myalgia, headache, photophobia, lymphadenopathy and skin rashes. The symptoms can progress to a subacute to chronic phase, where joint manifestations and fatigue persist. A bibliographic search was carried out in the PubMed, Cochrane, Dovepress and SciELO databases, using the keywords: chikungunya and chikunguña, crossed with the words: chronic arthritis and treatment. A total of 16 articles were included: 6 review articles, 1 systematic review, 4 original articles, 2 clinical trials, 1 letter to Editor letter and 2 case reports. Among the pharmacological options, methotrexate is the most studied drug and competes with the effects of NSAIDs. Non-pharmacological therapies are physiotherapy and physical therapy, used as complements to pharmacological therapy. The use of home therapies, including homeopathy, could also be added, mainly addressing the psychological and cultural factors of the patients.

ABSTRACT

El virus chikungunya (CHIKV) es un alfavirus cuya infección provoca una enfermedad caracterizada principalmente por fiebre y dolores articulares/musculares. Entre 25-50% de las infecciones se presentan con enfermedad crónica que puede durar de meses a años. El primer brote de CHIKV en Paraguay corresponde al año 2015, siendo el último en el año 2022/2023. Diversos candidatos vacunales contra CHIKV se encuentran en diferentes etapas de desarrollo, e incluso recientemente (noviembre/2023) fue aprobada la primera vacuna contra CHIKV llamada VLA1553 (Ixchiq). Adicionalmente, al menos 30 candidatos vacunales se encuentran en ensayos preclínicos/clínicos. Con la aprobación de la primera vacuna contra CHIKV y la posibilidad de otras que lleguen al mercado prontamente, debido al estado avanzado de otros candidatos vacunales, se abrirá un nuevo escenario en esta enfermedad. Se espera que la introducción de vacunas efectivas genere un avance importante para la prevención de esta enfermedad, disminuyendo los casos agudos y los efectos crónicos de la infección por el virus. En este trabajo de revisión se analiza el avance de las vacunas contra CHIKV, además de examinar los desafíos de vigilancia epidemiológica que plantean la introducción de estas vacunas.


Chikungunya virus (CHIKV) is an alphavirus that causes an illness characterized mainly by fever and joint/muscle pain. Between 25-50% of infections present with chronic diseases that can last from months to years. The first outbreak of CHIKV in Paraguay occurred in 2015, with the last outbreak occurring in 2022/2023. Several vaccine candidates against CHIKV are in different stages of development, and even recently (November/2023), the first vaccine against CHIKV, called VLA1553 (Ixchiq), was approved. In addition, at least 30 vaccine candidates are available for preclinical and clinical trials. With the approval of the first vaccine against CHIKV and the possibility of others coming to the market soon, due to the advanced status of other vaccine candidates, a new scenario will open for this disease. The introduction of effective vaccines is expected to generate an important advance in the prevention of this disease, reducing acute cases and the chronic effects of viral infection. This review analyzes the progress of CHIKV vaccines and examines the epidemiological surveillance challenges posed by the introduction of these vaccines.

ABSTRACT

Las manifestaciones típicas de la Chikungunya son la fiebre y las artralgias en la mayoría de los casos, pero debemos recordar que presentan manifestaciones mucocutáneas, donde la más frecuente es el exantema, además pueden presentar manifestaciones mucocutáneas atípicas con un gran polimorfismo simulando varias dermatosis como los casos expuestos anteriormente. Este reporte cuenta con 5 casos clínicos del mes de enero y febrero del año 2023 en el departamento Central de Paraguay, durante la mayor epidemia de Chikungunya registrada en el país hasta el momento. Es importante conocerlas y correlacionar con los demás signos y síntomas de la enfermedad para un diagnóstico y tratamiento adecuados.


The typical manifestations of Chikungunya are fever and arthralgia in most cases, but we must remember that they present mucocutaneous manifestations, where the most frequent is rash, they can also present atypical mucocutaneous manifestations with great polymorphism simulating various dermatoses such as cases outlined above. This report has 5 clinical cases from the month of January and February 2023 in the Central area of Paraguay, during the largest recorded Chikungunya epidemic in the country so far. It is important to know them and correlate them with the other signs and symptoms of the disease for proper diagnosis and treatment.

ABSTRACT

Abstract Dengue, zika, and chikungunya are arboviruses of great epidemiological relevance worldwide. The emergence and re-emergence of viral infections transmitted by mosquitoes constitute a serious human public health problem. The neurological manifestations caused by these viruses have a high potential for death or sequelae. The complications that occur in the nervous system associated with arboviruses can be a challenge for diagnosis and treatment. In endemic areas, suspected cases should include acute encephalitis, myelitis, encephalomyelitis, polyradiculoneuritis, and/or other syndromes of the central or peripheral nervous system, in the absence of a known explanation. The confirmation diagnosis is based on viral (isolation or RT-PCR) or antigens detection in tissues, blood, cerebrospinal fluid, or other body fluids, increase in IgG antibody titers between paired serum samples, specific IgM antibody in cerebrospinal fluid and serological conversion to IgM between paired serum samples (non-reactive in the acute phase and reactive in the convalescent). The cerebrospinal fluid examination can demonstrate: 1. etiological agent; 2. inflammatory reaction or protein-cytological dissociation depending on the neurological condition; 3. specific IgM, 4. intrathecal synthesis of specific IgG (dengue and chikungunya); 5. exclusion of other infectious agents. The treatment of neurological complications aims to improve the symptoms, while the vaccine represents the great hope for the control and prevention of neuroinvasive arboviruses. This narrative review summarizes the updated epidemiology, general features, neuropathogenesis, and neurological manifestations associated with dengue, zika, and chikungunya infection.


Resumo Dengue, zika e chikungunya são arboviroses de grande relevância epidemiológica em todo o mundo. A emergência e reemergência dessas infecções virais transmitidas por mosquitos constituem um grave problema de saúde pública humana. As manifestações neurológicas causadas por esses vírus têm alto potencial de morte ou sequelas. As complicações que ocorrem no sistema nervoso associadas às arboviroses podem representar um desafio diagnóstico e de tratamento. Em áreas endêmicas, casos suspeitos devem incluir encefalite, mielite, encefalomielite, polirradiculoneurite e/ou outras síndromes do sistema nervoso central ou periférico, na ausência de explicação conhecida. Caso confirmado de arbovirose neuroinvasivo é baseado na detecção viral (isolamento ou RT-PCR) ou de antígenos em tecidos, sangue, líquido cefalorraquidiano ou outros fluidos corporais, aumento dos títulos de anticorpos IgG entre amostras de soro pareadas, anticorpo IgM específico no líquido cefalorraquidiano e conversão sorológica para IgM entre amostras de soro pareadas. O exame do líquido cefalorraquidiano pode demonstrar: 1. agente etiológico; 2. reação inflamatória ou dissociação proteico-citológica, dependendo do quadro neurológico; 3. valor absoluto de IgM específica; 4. síntese intratecal de anticorpos IgG específicos (dengue e chikungunya); 5. exclusão de outros agentes infecciosos. O tratamento das complicações neurológicas visa melhorar os sintomas, enquanto a vacina representa a grande esperança para o controle e a prevenção das arboviroses neuroinvasivas. Esta revisão narrativa resume a atualização da epidemiologia, características gerais, neuropatogênese e manifestações neurológicas associadas à infecção pelos vírus da dengue, zika e chikungunya.

Introducción: La infección aguda por chikungunya produce una infección auto-limitada caracterizada principalmente por fiebre alta, artralgias, artritis, cefalea, mialgias y exantema pruriginoso; una minoría presenta síntomas persistentes que puede evolucionar a la cronicidad. Objetivo: describir las características clínicas y laboratoriales de pacientes adultos ambulatorios con chikungunya en un hospital de referencia durante la epidemia del 2022-2023 en Paraguay. Metodología: estudio descriptivo de corte trasversal de pacientes adultos con chikungunya que consultaron en el Hospital Nacional de Itauguá entre enero-marzo de 2023. Resultados: Se incluyeron 140 pacientes entre 19 y 79 años (edad media: 46,6±13,6), 77,9 % del sexo femenino y 89,3 % del departamento Central. El 72,9 % de los pacientes fueron confirmados por RT-PCR y el resto por la serología o nexo epidemiológico. El 48,6 % tenía alguna comorbilidad, el 65% se encontraba en la fase aguda, y el resto en la fase subaguda (35 %). Los síntomas más frecuentes fueron la poliartralgia (96,4 %); artritis (98,6 %), fiebre (95,7 %), astenia (100 %), mialgias (96,4 %), cefalea (82,9 %), exantema (95 %). Los síntomas gastrointestinales fueron menos frecuentes; náuseas (37 %), vómitos (15,7 %) y diarrea (20 %). Entre los principales hallazgos laboratoriales se encontraron la proteína C reactiva aumentada (45,7 %), leucopenia (23,6 %), eritrosedimentación aumentada (27,9 %), citólisis hepática leve (13,6 %). Todos fueron tratados con paracetamol; el 60,4 % y el 87,8 % de los pacientes en fase aguda y subaguda recibieron AINES, respectivamente. Discusión: La combinación de fiebre alta y artralgia severa fue el rasgo característico de la infección por el virus chikungunya en la epidemia en Paraguay.


Introduction: Acute infection by chikungunya produces a self-limited infection characterized mainly by high fever, arthralgia, arthritis, headache, myalgia and pruritic rash; a minority presents persistent symptoms that can evolve to chronicity. Objective: to describe the clinical and laboratory characteristics of adult ambulatory patients with chikungunya at a referral hospital during the 2022-2023 epidemic in Paraguay. Method: descriptive cross-sectional study of adult patients with chikungunya infection who consulted at the Hospital National de Itauguá between January-March 2023. Results: 140 patients between 19 and 79 years old (mean age: 46.6 ± 13.6), 77.9% female and 89.3% from the Central department, were included. 72.9% of the patients were confirmed by RT-PCR and the rest by serology or epidemiological nexus. 48.6% had some comorbidity, 65% were in the acute phase, and the rest in the subacute phase (35%). Increased C-reactive protein (45.7%), leukopenia (23.6%), increased erythrocyte sedimentation rate (27.9%), and mild hepatic cytolysis (13.6%) were among the main laboratory findings. All patients were treated with paracetamol; 60.4% and 87.8% of the patients in the acute and subacute phase, respectively, received NSAIDs. Discussion: The combination of high fever and severe arthralgia can be considered the characteristic feature of chikungunya virus infection in this epidemic.

Los principales desafíos de esta enfermedad son el abordaje multidisciplinario en la gestión del manejo del vector, la evaluación de las causas de esta alta letalidad y la necesidad de una vacuna de uso poblacional.


During the summer of 2022 and 2023, the largest Chikungunya epidemic occurred in Paraguay, and one of the largest reported in the region. It was centralized in the metropolitan area of Asunción in a first stage, but it expanded to the rest of the country during the first months of 2023. This work has the objective of describing the epidemic epidemiologically and clinically from its beginning in epidemiological week 40 of 2022 to week 20 of 2023. Methodology: it is a descriptive study that uses the public data available on the page of the General Directorate of Health Surveillance. 86,761 cases were confirmed, 58% female, 8,227 hospital admissions, and 248 deaths. The global lethality is 2.8 per thousand confirmed cases. The most affected age groups correspond to the 0 to 4-year-old group (9%), however, the highest incidence of cases occurs in people over 80 years of age. The main challenges of this disease are the multidisciplinary approach in the management of the vector, the evaluation of the causes of this high lethality and the need for a vaccine for population use.

ABSTRACT BACKGROUND AND OBJECTIVES: Particular pain features, such as pain interference, neuropathic-like symptoms, and central sensitization (CS) symptoms may be present in patients with Chikungunya fever and lead to functional limitations. The present study aimed to assess the association between pain characteristics and the disability in participants affected by Chikungunya fever in the chronic phase. METHODS: A cross-sectional study was conducted with 36 participants who filled out a sociodemographic, pain characteristics (pain interference - Brief Pain Inventory, neuropathic-like symptoms - PainDETECT Questionnaire, and CS-related signs and symptoms - Central Sensitization Inventory) and disability (Health Assessment Questionnaire) questionnaires. The Spearman correlation test (rho) verified the relationship between the outcomes. RESULTS: Most of the participants were female (77%), with a mean age of 43 years. Twenty-seven (75%) participants presented nociceptive pain and 11 (30%) had central sensitization symptoms. There was a high positive correlation between the presence of neuropathic-like symptoms and disability (rho=0.71; p<0.001) and pain intensity and disability (rho=0.76; p<0.001). A moderate positive correlation was found between the central sensitization symptoms and disability (rho=0.51; p=0.002). Moreover, there is a low positive correlation between pain interference in an individual's life and disability (rho=0.34; p=0.041). CONCLUSION: Patients in chronic phase of Chikungunya fever revealed mild pain intensity and predominance of nociceptive pain. Pain interference, neuropathic-like symptoms, and central sensitization symptoms negatively impact individual's disability after Chikungunya fever.


RESUMO JUSTIFICATIVA E OBJETIVOS: Características particulares da dor, como interferência da dor, sintomas do tipo neuropático e sintomas de sensibilização central (SC), podem estar presentes em pacientes com febre Chicungunha e levar a limitações funcionais. O presente estudo teve como objetivo avaliar a correlação entre as características da dor e a capacidade funcional em participantes acometidos pela febre Chicungunha na fase crônica. MÉTODOS: Foi realizado um estudo transversal com 36 participantes que preencheram questionários sociodemográficos, de características de dor (interferência da dor - Inventário Breve de Dor, sintomas do tipo neuropático - questionário PainDETECT, e sinais e sintomas relacionados à SC - Inventário de Sensibilização Central) e de capacidade funcional (Health Assessment Questionnaire). O teste de correlação de Spearman (rho) verificou a relação entre os desfechos. RESULTADOS: A maioria dos participantes era do sexo feminino (77%), com média de idade de 43 anos. Vinte e sete (75%) participantes apresentaram dor nociceptiva e 11 (30%) apresentaram sintomas de sensibilização central. Houve alta correlação positiva entre a presença de sintomas do tipo neuropático e capacidade funcional (rho=0,71; p<0,001) e intensidade da dor e capacidade funcional (rho=0,76; p<0,001). Foi encontrada uma correlação positiva moderada entre os sintomas de sensibilização central e a capacidade funcional (rho=0,51; p=0,002). Além disso, há uma correlação positiva baixa entre a interferência da dor na vida do indivíduo e a capacidade funcional (rho=0,34; p=0,041). CONCLUSÃO: Pacientes em fase crônica da febre Chicungunha apresentaram intensidade de dor leve e predominância de dor nociceptiva. A interferência da dor, os sintomas do tipo neuropático e os sintomas de sensibilização central afetam negativamente a capacidade funcional do indivíduo após a febre Chicungunha.

ABSTRACT

Introducción: La fiebre chikungunya (FCHIK) es una enfermedad emergente transmitida por mosquitos y causada por un alfavirus, el virus chikungunya (CHIKV). Aunque la mayoría de las personas se recuperan completamente en pocos días o semanas, algunos pacientes requieren hospitalización, pudiendo incluso desarrollar manifestaciones graves e incluso observarse un desenlace fatal. El objetivo del presente trabajo de investigación es describir las características de los pacientes atendidos en el Instituto de Medicina Tropical (IMT), en el marco de la actual Epidemia de Arbovirosis por Chikungunya. Materiales y Métodos: observacional, descriptivo, transversal con componente analítico, de pacientes de ambos sexos, atendidos en el IMT en sala de urgencia y en el área de hospitalización, con diagnóstico Confirmado o Sospechoso de FCHIK. Los datos fueron obtenidos de los sistemas informáticos del hospital (HIS). Resultados: Durante el periodo I (junio a octubre, 2022) fueron atendidos 19.645 pacientes (promedio de 3929 consultas mensuales) y durante el periodo II (noviembre 2022 a marzo 2023) 27.673 pacientes (promedio de 5.534,6 consultas mensuales); reflejando un incremento del 41% de consultas. El número de pacientes que requirieron hidratación y analgesia endovenosa fue incrementándose, con énfasis en los meses de enero, febrero y marzo (590, 781 y 608 casos, respectivamente). requiriendo consecuentemente el incremento de camas de hospitalización de 5 a 15 y de sillones de hidratación, de 10 a 15 sillones. Por otro lado, el número de pacientes que requirieron hospitalización se incrementó de 2 pacientes en el mes de noviembre a 54 pacientes en el mes de marzo. La letalidad fue significativamente superior en los >15 años, comparados con los ≤15 años: 12,5% (7/56) vs 2.9% (2/69), respectivamente (p<0.03. OR=4,8. (IC95%. 1 - 24.0). Conclusión: La epidemia de Chikungunya tuvo un importante impacto en la atención médica en el Instituto de Medicina Tropical, con un aumento significativo en la asistencia de pacientes febriles, traducida en alta tasa de pacientes atendidos en áreas de urgencias, y una pequeña proporción en área de internación. Este estudio subraya la importancia de una preparación adecuada y una respuesta rápida en el contexto de una epidemia para minimizar la repercusión en los servicios de atención médica hospitalaria.


Introduction: Chikungunya fever (CHIKF) is an emerging mosquito-borne disease caused by an alphavirus, chikungunya virus (CHIKV). Although most people fully recover in a few days or weeks, some patients require hospitalization and may even develop serious manifestations and even be fatal. The objective of this research work is to describe the characteristics of the patients treated at the Institute of Tropical Medicine (IMT), within the framework of the current Chikungunya Arbovirosis Epidemic. Materials and Methods: observational, descriptive, cross-sectional with an analytical component, of patients of both sexes, treated at the IMT in the emergency room and in the hospitalization area, with a confirmed or suspected diagnosis of FCHIK. Data were obtained from the hospital computer systems (HIS). Results: During period I (June to October 2022), 19,645 patients were attended (average of 3,929 monthly consultations) and during period II (November 2022 to March 2023) 27,673 patients (average of 5,534.6 monthly consultations); reflecting a 41% increase in inquiries. The number of patients who required intravenous hydration and analgesia increased, with emphasis on the months of January, February, and March (590, 781, and 608 cases, respectively). consequently requiring the increase of hospitalization beds from 5 to 15 and hydration chairs, from 10 to 15 chairs. On the other hand, the number of patients requiring hospitalization increased from 2 patients in November to 54 patients in March. This study underscores the importance of adequate preparation and rapid response in the context of an epidemic to minimize the impact on hospital medical care services.

ABSTRACT

Spontaneous mutations and lack of replication fidelity in positive-sense single stranded RNA viruses (+ssRNA virus) result in emergence of genetic variants with diverse viral morphogenesis and surface proteins that affect its antigenicity. This high mutability in +ssRNA viruses has induced antiviral drug resistance and ability to overcome vaccines that subsequently resulted in rapid viral evolution and high mortality rate in human and livestock. Computer aided vaccine design and immunoinformatics play a crucial role in expediting the vaccine production protocols, antibody production and identifying suitable immunogenic regions or epitopes from the genome sequences of the pathogens. T cell and B cell epitopes can be identified in pathogens by immunoinformatics algorithms and methods that enhance the analysis of protective immunity, vaccine safety, immunity modelling and vaccine efficacy. This rapid and cost-effective computational vaccine design promotes development of potential vaccine that could induce immune response in host against rapidly mutating pathogens like +ssRNA viruses. Epitope-based vaccine is a striking concept that has been widely employed in recent years to construct vaccines targeting rapidly mutating +ssRNA viruses. Therefore, the present review provides an overview about the current progress and methodology in computeraided vaccine design for the most notable +ssRNA viruses namely Hepatitis C virus, Dengue virus, Chikungunya virus and Coronaviruses. This review also highlights the applications of various immunoinformatics tools for vaccine design and for modelling immune response against +ssRNA viruses.

Introducción: La fiebre chikungunya (FCHIK) es una enfermedad febril transmitida por mosquitos, que en el niño tiene manifestaciones clínicas diferentes, dependiendo del grupo de edad. Objetivo: el presente trabajo tiene el propósito de identificar las características clínicas y evolutivas de la FCHIK en pacientes ≤15 años, que acudieron al Instituto de Medicina Tropical (IMT). Materiales y método: estudio observacional, descriptivo retrospectivo, basado en revisión de historias clínicas de pacientes de 0 y 15 años, que consultaron al IMT, entre noviembre/2022 y marzo/ 2023, con diagnóstico de FCHIK. Resultados: de 2050 pacientes febriles que consultaron al IMT entre noviembre/2022 y marzo/2023, 301 fueron casos de FCHIK, 10,6% (32/301) requirió hospitalización, de estos el 62,5% (20/32) era ≤1 año. La presencia de vómitos (p<0.0001.OR= 5,5 IC95% 2.2-13.2), dolor abdominal (p<0.0001.OR=40.5,5 IC95% 4.5-364.3); erupción maculo-papular-ampollar p<0.008. OR=3.5, IC95% 1.3 - 9.1), fue significativamente más frecuente en los hospitalizados. El 37.5% (12/32) fue hospitalizado en UCI y 15,6% (5/32) requirió ARM, 18.7%(6/32) presentó miocarditis; 15.6%(5/32) encefalitis, y 3.1%(1/32) encefalitis y miocarditis. No se registraron óbitos. Conclusiones: La décima parte de los pacientes con FCHIK requirieron hospitalización, de estos más de la mitad fueron <1 año. Las manifestaciones clínicas más frecuentes fueron fiebre, exantema, vómitos, irritabilidad, artralgias, lesiones ampollares. La leucopenia fue significativamente más frecuente en ≤1 año. La edad <3 meses, así como vómitos, exantema vesículo-ampollar, presencia de choque, irritabilidad, convulsiones, dolor abdominal y dificultad respiratoria se asociaron significativamente al requerimiento de hospitalización. Las complicaciones más frecuentes fueron el choque, la miocarditis y la encefalitis.

Materials and methods: this was a retrospective, descriptive and observational study, based on the review of medical records of patients between 0 and 15 years of age, who were evaluated at the ITM, between November/2022 and March/2023, who were diagnosed with CHKF. Results: of 2050 febrile patients who consulted at the ITM between November/2022 and March/2023, 301 were cases of CHKF, 10.6% (32/301) required hospitalization, of which 62.5% (20/32) were ≤ 1 year of age. The presence of vomiting (p<0.0001, OR= 5.5, 95% CI 2.2-13.2), abdominal pain (p<0.0001, OR=40.5.5, 95% CI 4.5-364.3); maculo-papular-bullous rash (p<0.008, OR=3.5, 95% CI 1.3 - 9.1), was significantly more frequent in hospitalized patients. 37.5% (12/32) were hospitalized in the ICU and 15.6% (5/32) required ARM, 18.7% (6/32) presented myocarditis; 15.6%(5/32) encephalitis, and 3.1%(1/32) had both encephalitis and myocarditis. No deaths were registered. Conclusions: One tenth of the patients with CHKF required hospitalization, of which more than half were <1 year of age. The most frequent clinical manifestations were fever, rash, vomiting, irritability, arthralgia, and bullous lesions. Leukopenia was significantly more frequent in those ≤1 year of age. Age <3 months, as well as vomiting, vesicular-bullous rash, shock, irritability, seizures, abdominal pain, and respiratory distress were significantly associated with the need for hospitalization. The most frequent complications were shock, myocarditis and encephalitis.

ABSTRACT

La fiebre chikungunya es una enfermedad viral descrita por primera vez en 1952. Actualmente se informan brotes esporádicos de fiebre chikungunya a nivel país (abril 2022). Presentamos el caso de un varón joven con fiebre chikungunya en fase subaguda la cual se presentó al ingreso hospitalario como fiebre de origen desconocido.


Chikungunya fever is a viral disease first described in 1952. Sporadic outbreaks of chikungunya fever are currently reported nationwide (April 2022). We present the case of a young man with chikungunya fever in the subacute phase which presented at hospital admission as fever of unknown origin.

ABSTRACT

Introdução: De acordo com a literatura científica, diagnósticos clínicos diferenciais de arboviroses representam uma dificuldade no que tange à dengue, na medida em que está no Brasil há muitos anos, o que acarreta em ser a arbovirose mais conhecida no país. As notificações de arboviroses se tornaram obrigatórias para inserção no SINAN, possibilitando a construção de perfis demográficos e o cálculo de incidências a partir de informações específicas para estas doenças. No que tange à dengue, a epidemia deste agravo ocorre no país desde 1986, evidenciando falhas na prevenção, relacionadas a aspectos socioeconômicos e ambientais. Objetivo: analisar perfis das notificações de dengue e febre de chikungunya dos casos notificados no município de Cabo Frio. Metodologia: Trata-se de estudo transversal e descritivo, com uso de dados secundários do SINAN referentes a casos de arboviroses no município de Cabo Frio/RJ. Foram observadas variáveis relacionadas ao sexo, escolaridade, raça/cor e critérios de confirmação, além do grau de completude. Resultados: Foram notificados 8.777 casos suspeitos de arboviroses, incluindo-se 1.367 notificações (15,57%) referentes à febre de chikungunya e 1.986 (22,63%), à dengue. Em relação ao desfecho, 1186 casos (51,45%) foram fechados como inconclusivos e 344 destes (14,92%) foram descartados como arboviroses. Dentre os inconclusivos, 943 (79,51%) eram referentes à notificação de dengue, idem para os 277 casos descartados (80,52%). Conclusão: Observou-se baixa taxa de completude nas fichas de notificação, explicada pelo baixo número de recursos humanos e pela insuficiente infraestrutura. Sugere-se a interação de diferentes profissionais e pesquisadores, facilitando a compreensão da complexa dinâmica das arboviroses em questão.


Introduction: According to the scientific literature, differential clinical diagnoses of arboviruses represent a difficulty with regard to dengue, as it has been present in Brazil for many years, which makes it the most well-known arbovirus in the country. Notifications of arboviruses became mandatory for inclusion in SINAN, enabling the construction of demographic profiles and the calculation of incidences based on specific information for these diseases. With regard to dengue, the epidemic of this disease has occurred in the country since 1986, showing failures in prevention, related to socioeconomic and environmental aspects. Objective: to analyze profiles of notifications of dengue and chikungunya fever of cases notified in the municipality of Cabo Frio. Methodology: This is a cross-sectional and descriptive study, using secondary data from SINAN regarding cases of arboviruses in the municipality of Cabo Frio/RJ. Variables related to sex, education, race/color and confirmation criteria were observed, in addition to the degree of completeness. Results: 8,777 suspected cases of arboviruses were reported, including 1,367 reports (15.57%) referring to chikungunya fever and 1,986 (22.63%) to dengue fever. Regarding the outcome, 1186 cases (51.45%) were closed as inconclusive and 344 of these (14.92%) were discarded as arboviruses. Among the inconclusive ones, 943 (79.51%) were related to dengue notification, the same for the 277 discarded cases (80.52%). Conclusion: A low completeness rate was observed in the notification forms, explained by the low number of human resources and insufficient infrastructure. It is suggested the interaction of different professionals and researchers, facilitating the understanding of the complex dynamics of the arboviruses in question.


Introducción: Según la literatura científica, los diagnósticos clínicos diferenciales de los arbovirus representan una dificultad con respecto al dengue, ya que está presente en Brasil desde hace muchos años, lo que lo convierte en el arbovirus más conocido en el país. Las notificaciones de arbovirus pasaron a ser obligatorias para su inclusión en el SINAN, lo que permitió la construcción de perfiles demográficos y el cálculo de incidencias a partir de información específica de estas enfermedades. Con respecto al dengue, la epidemia de esta enfermedad se presenta en el país desde 1986, mostrando fallas en la prevención, relacionadas con aspectos socioeconómicos y ambientales. Objetivo: analizar perfiles de notificaciones de dengue y fiebre chikungunya de los casos notificados en el municipio de Cabo Frio. Metodología: Se trata de un estudio transversal y descriptivo, utilizando datos secundarios del SINAN sobre casos de arbovirus en el municipio de Cabo Frio/RJ. Entre los inconclusos, 943 (79,51%) estaban relacionados con la notificación de dengue, lo mismo para los 277 casos descartados (80,52%). Conclusión: Se observó un bajo índice de completitud en los formularios de notificación, explicado por el bajo número de recursos humanos y la infraestructura insuficiente. Se sugiere la interacción de diferentes profesionales e investigadores, facilitando la comprensión de la compleja dinámica de los arbovirus en cuestión.

ABSTRACT

Introdução: A tripla circulação de arboviroses no cenário nacional, traz uma constante preocupação para a saúde pública. Relato de caso: Gestante, 38 anos, primigesta, apresenta infecção aguda com quadro de febre, artralgia e mialgia, alteração de níveis pressóricos, elevação de enzimas hepáticas, plaquetopenia e elevação de LDH que gerou quadro grave, resultando em parto de emergência e hemorragia pós-parto maciça. Após suspeição e realização de exames sorológicos, foi constatado codetecção por dengue e chikungunya. Considerações finais: A codetecção de dengue aguda e chikungunya gerou quadro de pré-eclâmpsia e hemorragia pós-parto. Após conduta oportuna, gestante e RN seguiram bem e em alta. Tal situação serve de alerta para vigilância epidemiológica.


Introduction: The triple circulation of arboviruses on the national scene is a constant concern for public health. Case report: Pregnant woman, 3 years old, primigravidae, presented with an acute infection with fever, arthralgia and myalgia, changes in blood pressure levels, elevated liver enzymes, thrombocytopenia and elevated LDH levels, which generated a severe condition, resulting in emergency delivery and hemorrhage. massive postpartum. After suspicion and serological tests, co-detection by dengue and chikungunya was confirmed. Final considerations: The co-detection of acute dengue and chikungunya generated preeclampsia and postpartum hemorrhage. After timely conduct, the pregnant woman and the NB continued well and were discharged. This situation serves as a warning for epidemiological surveillance.

ABSTRACT Introduction: Chikungunya chronic joint disease causes debilitating arthralgia, significantly impacting the quality of life of affected individuals. Methods: In this study, patients underwent clinical follow-ups, joint biopsies, and pre-biopsy and 24 months post-biopsy serum dosage of cytokines. Results: All participants were female and had pain in 12 joints on average, with 41.17% exhibiting moderate disease activity. Histopathological analysis revealed collagen deposition. Indirect immunofluorescence detected the CHIKV glycoprotein E1 antigen, and an increase in cytokines. Conclusions: Persistent inflammation and ineffective antiviral immune responses leading to antigen persistence may contribute to chronic CHIKV arthritis.
